For the purpose and in compliance with the Data Privacy Act of 2012 (“Act”), the “Personal Information Controller” or PIC is Converge Information and Communications Technology Solutions, Inc. with its main office registered in # New Street Bldg., Mc Arthur Highway, Balibago, Angeles City, Pampanga.

You have choices about the data we collect. You may decline when you are asked to provide personal information, but if you choose not to provide any personal information that is required by this Appointment System (“Web Application”), you may not be able to proceed with the registration and booking an appointment with us.

The personal information that we collect about you may include your Subscriber Account Number, Full Name, Mobile Number, Email Address and Birth Date for purposes of (1) recognizing that you are indeed a legitimate CNVRG subscriber, (2) sending a verification link to your registered email and mobile number to ensure that the registration transaction is secured and protected, (3) providing you timely notification about your chosen schedule of visit, and (4) managing travel restrictions in adherence to Inter-Agency Task Force’s (IATF) community quarantine protocols.

We adhere to the principle of proportionality and CNVRG will only collect personal information necessary to fulfill the purpose of this undertaking. We will not collect any personal information that are irrelevant and does not serve any purpose.

How we collect your information?

In order to book an appointment with us and continue availing our products and services, CNVRG will collect and process your personal information, as stated in “What information we may collect?“ section of this Privacy Notice.

We ensure that your personal information is secured and protected in accordance with this Privacy Notice and in compliance with Act, its Implementing Rules and Regulations (IRR), its issuances and notices and applicable data protection laws. We will keep your personal information for as long as necessary.

How we protect your information?

By submitting your personal information, you agree to the processing which includes collection, usage, retention, and storage of your personal information at our authorized and secured database locations.

All personal information that you have provided us is transmitted using Secure Socket Layer (SSL) encryption technology and will be stored on our secured servers.

In addition to the steps that we take to protect your personal information, you should follow these precautionary measures to help secure yourself against unauthorized disclosure or fraud:

• choose a password you can remember but others will not guess, and if you do write it down, keep it somewhere safe and secure;
• when you have finished any session on our Web Application, make sure that you prevent your details from being seen by any third parties or unauthorized persons, and if you have registered and logged in, remember to log off; and
• follow the password guidelines specified in the Web Application.

Our Web Application may, from time to time, contain links to and from the different websites of CNVRG. If you follow a link to any of these websites, please note that these websites have their own Privacy Notices in place. Please check the Privacy Notice before you submit any personal information on our websites.

We adhere to the Act and its IRR, and it is our obligation to ensure that proper security controls are in place and are effectively implemented through our information security and data privacy policies and procedures. While the transfer of information via the Internet may not be completely secure, we will do our best to protect and control your personal information from the time we receive it. We have enforced adequate organizational and technical security measures to appropriately safeguard your personal information.

How we use your information?

We use the personal information that we have collected from you for these basic purposes:

1. To process your submitted appointment application and prepare us prior to your visit.
2. To assist your concerns, queries, or complaints about the services that we offer, and for us to improve our products and services.
3. To review whether the booked appointment does not impact, in any way the community quarantine restrictions implemented by authorities.
4. To validate and authenticate you as the actual subscriber who have registered and will be using this Web Application accordingly.

How we share your information?

We will not share or disclose to anyone the personal information you provided us. All personal information shall be and will be processed only by authorized CNVRG personnel. We will collect and process your personal information to comply with the Act and its IRR or any applicable laws to respond to valid legal process, including law enforcement and government agencies. We shall also use your personal information necessary to protect the rights or property of CNVRG, including enforcing the terms governing the use of the services.
